Policies and Procedures

Office hours are as follows:

Mon thru Thurs, 8:00a.m. to 4:30 p.m.

Friday 8:00 a.m. to 1:00 p.m.

Patients are seen by appointment only

You may call for an appointment during our office hours stated above. . Our telephone number is (423) 756-4796. Please inform the receptionist which doctor you would like to see and what type of appointment you requesting. For example, gynecological exam, obstetric exam, etc. Please let us know if you are a new or established patient. We will also ask for your insurance information so that we may determine whether we are participating with your insurance company. Please verify with your insurance company before your appointment whether the doctor you have chosen to see is participating with your insurance.

If you are a new patient, we will obtain your address so we can mail the new patient information and medical questionnaire to you prior to your appointment. Please fill these forms out completely and bring with you the day of your appointment. Please arrive a few minutes before your appointment time to allow us to create a chart and copy your information. Please remember to bring your insurance card!!

If you have been referred by another physician and your insurance requires you to have a referral, please make sure the referral is received in our office before your appointment date. If you are seen without a referral, you will be responsible for payment at the time you are seen, or we can reschedule your visit.

In addition to the above, Dr. Breen does not participate with the following:
John Deere TennCare
TennCare Select

We will gladly file your insurance for you, however, we do request that your portion (co-payment, deductible, co-insurance) be paid at the time services are rendered. These payments may be made by cash, check, or credit card.

(We accept, Visa, MasterCard, American Express, and Discover.)

**Again, it is imperative that you make sure we are participating with your insurance before being seen in our office. If we are not participating, you will be responsible for payment.**
